I'm using Chrome and it's working for me as soon as I click Save.

I was able to recreate this situation. Even if the global element is set on page 2 it still doesn't show in InteractMode.

When you go to InteractMode did you chose the option: All pages, starting from 1. You should be able to see if the app is syncing with the server, in the bottom right corner, after you select that option. 

Could you redo the Draw behavior and see if you are still having this problem?

You can change your security preferences to let you install apps from unidentified developers and then it should let you install InteractBuilder.

After you log in, and click your app it should load it in InteractMode.

I found a small tutorial on youtube that explains how to export files from InDesing to Photoshop using a custom script:

It seems you need to export your InDesign file as a PDF file then you can open that PDF file in Photoshop and then save it as a PSD file.
